Bank Change Request

Please submit the following information to indyoperations@incommhealthcare.com. 

  • Business Name
  • Principal Name/Owner (the person that signed the original contract)
  • Address
  • Contact Phone Number
  • Merchant ID (MID) – You can find this on your daily remittance report. It is a five-digit numerical ID that precedes your terminal ID XXXXXMDT

*InComm requires signature from the original principal owner for the request, no exceptions. If you are a new owner and wish to apply for services, please email otcapplication@incomm.com. Someone will contact you and provide you with an application to complete.

Change of Ownership

If you are the existing owner, please send the following information to indyoperations@incommhealthcare.com.

  • Business Name
  • Principal Name/Owner
  • Address
  • Contact Phone Number
  • Merchant ID (MID) –You can find this on your daily remittance report. It is a five-digit numerical ID that precedes your terminal ID XXXXXMDT
  • The date your store will be closing under your ownership – this will be the date your terminal will become inactive

*The new owner must apply for services and gain approval to start processing OTC transactions. Terminals are non-transferable. If the new owner wishes to apply, they can do so by sending an email to otcapplication@incomm.com. The new owner must wait to receive their terminal to begin processing transactions.
